This was a highly complex case because the patient had been missing teeth for many years, had significant bone loss in both jaws and had worn removable dentures for a long time. In the lower jaw, the mental foramen was close to the ridge and the soft tissue had many fibrous bands, making flap elevation and nerve preservation difficult. The upper jaw had D4 bone quality, and a sinus perforation occurred at the tooth 16 site during surgery. We adjusted the implant positions and combined bone augmentation with a barrier membrane to ensure stability and safety. With an appropriate treatment plan, the implants were placed exactly as planned, creating a good foundation for a fixed restoration and long-term chewing function.

Loss of almost all teeth in both jaws
The remaining teeth had large cavities and fractures, severe wear, heavy tartar and external staining, with a poor prognosis for preservation.
The patient had worn removable dentures for many years, with clear ridge resorption affecting chewing and appearance.

Diagnosis

Mất nhiều răng, các răng còn lại tiên lượng bảo tồn kém.
Tiêu xương ổ răng mức độ nhiều do mất răng lâu năm và sử dụng hàm tháo lắp.
Hàm dưới mất nhiều răng còn 6 răng cửa trước, mật độ xương (D1–D2)
Hàm trên có mật độ xương D4, chiều cao xương vùng răng sau hạn chế, cần lựa chọn hướng đặt Implant phù hợp và ghép xương bổ sung tại vị trí cần thiết.
Chức năng ăn nhai và thẩm mỹ suy giảm nghiêm trọng.

2. Treatment Plan

Challenges

Lower jaw: Significant bone loss, a mental foramen close to the ridge and soft tissue with many fibrous bands, making flap elevation and nerve location difficult.
Upper jaw: Poor D4 bone quality and a sinus perforation at the tooth 16 site during flap elevation, so a membrane was placed and the implant position was changed to ensure safety.

Giai đoạn 1: Xử lý mô viêm

Nhổ các răng còn lại không còn khả năng bảo tồn. Làm sạch mô viêm, xử lý nền hàm.

Giai đoạn 2: Cấy implant hàm trên

Cấy Implant hàm trên sử dụng 4 Implant Neo II, gia tăng thể tích xương vùng thiếu hụt

Giai đoạn 3: Cấy implant hàm dưới

Đặt 4 implant hàm dưới, tại các vị trí R32, R42, R34, R44

Cấy implant hàm dưới

- Nhổ các răng còn lại không còn khả năng bảo tồn. - Cấy 04 Implant Neo II, kích thước Implant Ø4.0 × 11 mm

Buổi 2

Cấy implant hàm trên

- Cấy 4 implant hàm trên - Gia tăng thể tích xương vùng thiếu hụt. - Đặt Cover Screw.

Buổi 3

Phục hình hàm tạm

- Phục hình hàm tạm trên Implant sau phẫu thuật. - Theo dõi quá trình lành thương và tích hợp xương.

Buổi 4

Lắp răng hoàn thiện

Sau 6 tháng, phục hình cố định toàn hàm trên Implant nhằm khôi phục chức năng ăn nhai và thẩm mỹ lâu dài.
