The patient came to us having lost almost all teeth; the remaining teeth were badly damaged with a poor prognosis, and there was bone loss at several sites. After clinical and CT assessment, we chose implants for both jaws, treating the upper jaw first to ensure a safe and effective process.

1. Treatment Profile

Triệu chứng & tình trạng

Loss of almost all upper and lower teeth, with only a few loose natural teeth remaining and no chewing ability.
Moderate bone loss in the upper jaw, especially in the back areas (16, 26) with limited bone height (6–7 mm).
The lower jaw bone was still relatively good, with 12–13 mm of height and 6–8 mm of width, favorable for implant placement.

Diagnosis

Mất răng toàn bộ hàm dưới và mất nhiều răng hàm trên, các răng còn lại hư hỏng nặng, không còn khả năng bảo tồn.
Viêm nha chu mạn tính kèm tích tụ nhiều cao răng và mảng bám, gây lung lay và suy giảm nâng đỡ các răng còn lại.
Tiêu xương ổ răng mức độ vừa đến nặng, rõ hơn ở vùng hàm trên.
Chức năng ăn nhai suy giảm nghiêm trọng, ảnh hưởng đến phát âm, thẩm mỹ và chất lượng cuộc sống.
Chỉ định nhổ các răng tiên lượng xấu và phục hồi bằng Implant hai hàm.

2. Treatment Plan

Stage 1: Upper jaw implant surgery, placement of 4 Neo 3 implants. Cover screws placed after surgery.
Stage 2: Once the upper jaw is stable, placement of 4 Neo 3 implants. Multi-unit abutments placed.
Stage 3: Fixed restorations for the upper and lower jaws. Check of chewing function and instructions on implant care.

Giai đoạn 1

Thăm khám và lập kế hoạch

Khám lâm sàng, chụp CT Cone Beam 3D. Đánh giá thể tích xương và lập kế hoạch cấy ghép hai hàm. Thực hiện hàm trên trước, sau đó mới cấy hàm dưới.

Giai đoạn 2

Cấy implant hàm trên

Cấy 4 Implant Neo 3 theo kế hoạch, có gia tăng thể tích xương. Đặt Cover Screw, theo dõi quá trình lành thương và tích hợp xương.

Giai đoạn 3

Cấy implant hàm dưới

Sau khi hàm trên ổn định, cấy 4 Implant Neo 3 hàm dưới. Đặt Multi-unit Abutment với lực siết trên 50 Ncm.

Giai đoạn 4

Phục hình và theo dõi

Lấy dấu chế tác hàm. Thử phục hình, điều chỉnh khớp cắn. Lắp răng cố định trên Implant
